Hospital at Home Initiative
The Hospital at Home (HaH) program is an innovative service aimed at improving patient outcomes and satisfaction by allowing eligible patients to receive acute-level care in the comfort of their homes instead of in a hospital setting.
How the Program Operates
Patients who meet specific criteria are transitioned to their homes, where they continue to receive essential acute care. Although their treatment occurs at home, they remain classified as inpatients. They benefit from daily in-person care provided by a dedicated interdisciplinary team of hospital clinicians, supplemented by virtual consultations and remote monitoring. Our nursing and physician services are available around the clock.
Our Hospital at Home nurses will have access to fleet vehicles to facilitate patient care.
Role of the Nurse Educator
We are seeking a Nurse Educator to join the Hospital at Home program at Providence Health Care. This role is crucial in enhancing acute care capacity, improving patient outcomes, and ensuring high levels of patient satisfaction. The Nurse Educator will play a vital part in this transformative approach to healthcare delivery.
As a Nurse Educator, your responsibilities will include:
- Acting as a clinical resource, demonstrating expertise and leadership while facilitating learning.
- Monitoring staff development against established growth plans and adjusting strategies as necessary to support their progress.
- Implementing diverse teaching and learning methodologies aligned with the identified growth and learning objectives.
- Engaging in research, quality improvement, and risk management initiatives as needed.
- Staying updated with advancements in practice, technology, and equipment to ensure educational content is current and relevant.
- Supporting nursing practices across the organization by participating in committees and working groups focused on educational program design and evaluation.
Qualifications and Skills
To be successful in this role, candidates should possess:
- Registration with the BC College of Nurses and Midwives as a practicing RN, along with three years of recent relevant nursing experience, including one year in teaching or leadership roles.
- Strong abilities in teaching, coaching, and providing clinical support.
- Knowledge of adult education principles and the ability to develop educational programs tailored to identified learning needs.
- Excellent communication skills, both verbal and written, and the ability to work collaboratively within an interdisciplinary team.
- A commitment to continuous quality improvement and the ability to apply analytical skills in decision-making.
