Position Overview
In the role of a Community Support Facilitator, you will serve as the main liaison for individuals with developmental disabilities and their families. Your responsibilities will include:
- Assessing eligibility for CLBC-funded services
- Facilitating access to community resources, informal support, and CLBC services
- Providing comprehensive information about available community and peer support services
- Addressing crisis situations and offering solutions
- Coordinating various services to meet individual needs
- Developing and executing personalized support plans that incorporate both formal and informal safeguards
- Helping individuals and families engage with their communities to leverage their strengths
- Guiding individuals and families in identifying necessary activities and resources to achieve their personal goals
Qualifications
- A Bachelor of Social Work or an equivalent qualification is preferred
- Experience in the community living sector, particularly with children, youth, or adults with developmental disabilities and their families
- Consideration will be given to candidates with a combination of relevant education and experience
A valid class 5 driver's license and a clean driving record are mandatory.
Additional Information
Team: NTC Pacific Northwest
Status: Auxiliary, with an anticipated workload of 70 hours bi-weekly. This position may be filled by regular-status staff as a temporary assignment.
Classification/Salary: Social Program Officer (Growth to R24) with a starting hourly wage of $39.30 to $41.65.
