Position Title: Patient Care Facilitator

Organization: Home and Community Care Support Services

The Patient Care Facilitator plays a crucial role in overseeing a diverse group of patients facing various health challenges. This position involves evaluating patient requirements, formulating care strategies, coordinating services, and connecting patients with community resources. As a member of the Community Care Team, the facilitator provides essential support to patients, families, caregivers, and other healthcare professionals. Furthermore, they may assist Primary Care teams to improve patient outcomes.

Key Responsibilities:

  • Perform thorough patient evaluations
  • Guide patients and caregivers in establishing objectives
  • Create personalized care strategies
  • Organize healthcare services
  • Enhance communication among healthcare team members
  • Maintain accurate documentation of patient cases

Qualifications and Experience:

  • Registered Nurse, Physiotherapist, Occupational Therapist, Social Worker, Speech Therapist, Dietitian
  • At least two years of relevant professional experience

Expertise:

  • Understanding of medical conditions, case management, and healthcare regulations

Competencies:

  • Strong communication, collaboration, and problem-solving skills
  • Proficient in computer applications, valid driver's license
  • Fluency in both English and French

Union Affiliation: OPSEU

Compensation: $40.409/hr. - $43.169/hr.

Work Schedule: Monday to Friday, 8:30am to 4:30pm
