Tri-County Mennonite Homes is dedicated to providing exceptional services to seniors and individuals with developmental disabilities. We operate multiple facilities, including Greenwood Court, Nithview Community, and Aldaview Services, all aimed at enhancing the quality of life for our residents.
Role Summary:
We are seeking a dedicated on-call physician to support our Resident Home Areas within Long-Term Care, which accommodates 33 residents. This role offers collaboration with our Medical Director and Nurse Practitioner to ensure comprehensive care.
Key Responsibilities:
- Conduct weekly visits to provide clinical care to assigned residents;
- Address both acute and chronic health needs, ensuring timely and accurate documentation;
- Identify opportunities for palliative and end-of-life care, collaborating with the interdisciplinary team;
- Work efficiently with the medical team to uphold the highest standards of resident care;
- Perform regular medication reviews and maintain updated health records;
- Participate in after-hours on-call duties;
- Engage in ongoing education related to long-term care, geriatric care, palliative care, and dementia care;
- Take part in physician peer review processes to enhance resident care.
Qualifications:
- Licensure: Must possess a valid College of Physicians and Surgeons of Ontario License.
Experience:
- Preferred experience in a Long-Term Care environment.
Knowledge, Skills, and Abilities:
- In-depth understanding of the Long-Term Care Homes Act, 2007, and related regulations;
- Proven leadership capabilities with strong verbal and written communication skills.
