About the Role
We are seeking a highly skilled and experienced Director of Care to join our team at Extendicare Guildwood Home in Scarborough, Ontario. As a key member of our medical team, you will be responsible for providing care to residents in accordance with the standards of practice and offering leadership and guidance to the personal care staff.
Key Responsibilities
- Develop and implement quality improvement initiatives, risk management systems, and data collection and analysis to ensure high-quality resident care.
- Lead the coordination and implementation of best practice and risk management initiatives related to resident care, including falls, restraint use, wound management, and Infection Control.
- Effectively manage the utilization of nursing supplies and maintain a system of care assessment, planning, implementation, and evaluation at the resident and unit program level.
- Support the RAI-MDS program and maintain an active presence in the Resident Home Areas, mentoring staff while overseeing the provision of quality resident care.
- Initiate investigative and corrective actions related to occupational health and safety and develop and monitor modified work programs for nursing personnel.
- Collaborate with the Director of Care on special projects, reviews, and other duties as assigned.
- Identify areas of improvement for staff development and work with the DOC to ensure overall effectiveness of the nursing department.
- Advocate for ethical and effective practice in all areas of the organization.
- Complete Performance Appraisals with the inputs of other members.
- Focusing on Resident Centered Care Needs as required by MOH LTC Home Legislation and Extendicare Policies.
- Actively participate in the palliative care program, goals of care, and advanced care planning.
Requirements
- Registered Nurse registered with College of Nurses of Ontario; BScN an asset.
- Minimum of three years experience in long term care; previous management experience an asset.
- Current knowledge of clinical issues for geriatric residents, infection control, principles of adult education, and quality improvement.
- Knowledge of RAI MDS is required.
