We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Durham Community Health Centre. As a key member of our interdisciplinary team, you will provide comprehensive, client-centered primary health care to community members within high-priority populations.
Key Responsibilities:- Provide primary care assessments, screenings, and referrals to community programs and services.
- Activate clinical pathways and connect clients with community-based and specialty services as indicated.
- Collaborate with clients as partners in their care, recognizing the social determinants of health and associated health risks.
- Contribute to the development, implementation, and evaluation of medical directives, policies, and procedures.
- Participate in case conferences, chart audits, and quality improvement activities.
- Ensure client care and caseload is managed to achieve the goals of Durham CHC's Multi-Sector Service Accountability Agreement.
- Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class, Primary care.
- Advanced preparation at the Masters level.
- Member of the Nurse Practitioners Association of Ontario or Registered Nurses Association of Ontario is an asset.
- Minimum of three years' experience working as a primary care NP with clients having complex needs.
- Experience working with priority populations, such as Newcomers and ACB communities, is considered an asset.
- Knowledge of relevant services and community supports in Durham Region is strongly preferred.
- Must hold a Prescribing Narcotics and Controlled Substances certificate endorsed by the CNO.
