Quick Response Occupational Therapist

We are seeking a highly skilled Occupational Therapist to join our team at Fraser Health. As an Occupational Therapist, you will play a critical role in providing high-quality patient care and facilitating timely discharges from acute care facilities to community care services.
Key Responsibilities- Provide occupational therapy services to patients through assessment, consultation, and interventions that promote functional independence and occupational performance.
- Assess patient care needs by completing various assessments, including home assessments, and advocate for patients with community agencies and funding sources.
- Collaborate with community service providers to provide advice to patients, families, and healthcare professionals regarding discharge needs.
- Facilitate timely exchange and provision of information regarding patient care between acute care facilities and community services.
- Participate in discharge planning rounds and facilitate timely discharge of patients, ensuring appropriate resources are in place.
- Complete pre-discharge home assessments to aid in transition and discharge planning of patients from hospital or prevent admission of clients to hospital.
- Identify environmental barriers in the home and make recommendations for modifications to ensure access for patients with disabilities.
- Prescribe and recommend specialized and adaptive aids and equipment to patients for activities of daily living.
- Educate patients, families, caregivers, and staff on independent living skills and occupational performance, use of environmental and equipment adaptations, body mechanics, lifts, and transfers.
- Act as a patient advocate by providing information and liaising with funding agencies and community resources.
- Complete and maintain complete documentation of patient information, as required.
- Participate in quality improvement activities of the program by providing input into the development of indicators, standards, practices, and procedures.
- Current full registration with the College of Occupational Therapists of British Columbia (COTBC).
- Two years' recent, related clinical experience in a community or home health setting, or an equivalent combination of education, training, and experience.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
- Demonstrated knowledge of current developments in occupational therapy.
- Demonstrated understanding of the scope of practice of other healthcare disciplines.
- Demonstrated knowledge of community resources.
- Ability to organize and prioritize workload.
- Ability to problem-solve and exercise judgment in professional practice.
- Demonstrated leadership and teaching skills.
- Ability to work effectively independently and in a team-based environment.
- Ability to communicate effectively, both verbally and in writing.
- Ability to deal effectively with conflict.
- Ability to operate related equipment, including applicable software applications.
