About the STOP HIV/AIDS Initiatives:
The STOP HIV Outreach Team is an intensive case management group composed of nurses, social workers, and outreach workers dedicated to assisting individuals living with HIV who face challenges in accessing care and adhering to treatment. Our goal is to transition clients to lower intensity resources as they gain the skills necessary for self-care.
Responsibilities of the HIV/AIDS Outreach Nurse at Vancouver Coastal Health include:
Collaborating within a multidisciplinary team to deliver outreach services to at-risk and marginalized populations. Offering frontline primary health care services, including immediate assessment, intervention, and treatment. Conducting pre- and post-test counseling for HIV antibody testing, providing results, and ongoing support tailored to the needs of clients. Delivering harm reduction education and counseling on issues related to Sexually Transmitted Infections (STIs) and HIV in various community settings. Distributing harm reduction supplies to those in need. Partnering with community organizations to reach populations at high risk for STDs and HIV. Coordinating with AIDS organizations to develop and implement educational workshops and programs targeting specific groups. Identifying health issues, behaviors, and barriers faced by at-risk populations. Engaging in collaborative efforts to devise innovative strategies to overcome challenges and participate in research initiatives as required.Qualifications
Education & Experience
Current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). A minimum of two (2) years of recent relevant clinical nursing experience in community-based or Indigenous-led organizations, providing services to clients at risk or living with HIV/AIDS. Certification in Reproductive Health (STIs) with the BCCNM, as necessary. A valid BC Driver's license and access to a personal vehicle for work-related purposes.Knowledge, Skills & Abilities
Comprehensive understanding of nursing theory and practice within a client-centered framework. Familiarity with community development and health promotion principles. In-depth knowledge of BCCNM Standards for Practice for registered nurses. Extensive knowledge of HIV/AIDS, STIs, and communicable disease control. Awareness of mental health issues and substance use treatment approaches. General understanding of Trauma Informed Practice and Harm Reduction strategies. Proven ability to promote Indigenous Cultural Safety. Capacity to provide culturally competent care across diverse populations. Knowledge of harm reduction strategies in the context of infectious diseases and substance use. Familiarity with community resources related to HIV/AIDS. Strong communication skills, both verbal and written. Ability to analyze situations, problem-solve, and facilitate resolutions. Demonstrated initiative, flexibility, and the ability to work independently and collaboratively. Skills in managing aggressive and suicidal behavior. Competence in identifying appropriate community resources for client needs. Adaptability to new or unexpected situations. Proficiency in de-escalation and conflict resolution techniques. Leadership and consultation abilities. Proficiency in CPR techniques. Capability to operate relevant equipment and software applications.
