POSITION OVERVIEW:
The Hospice Care Coordinator is tasked with the recruitment and leadership of the nursing care team dedicated to delivering compassionate care and services to Residents. This role involves effective communication with Residents' families, healthcare providers, and regulatory bodies. The Coordinator is responsible for overseeing the nursing team, providing necessary guidance, direction, and discipline to ensure that care is tailored to the unique needs and care plans of each Resident. The Coordinator exemplifies leadership and gerontological best practices in alignment with provincial regulations and Park Place policies.
KEY RESPONSIBILITIES:
- Recruit, orient, and supervise the nursing care team to guarantee effective and comprehensive care planning and delivery, respecting Residents' choices and adhering to Park Place policies.
- Provide leadership through a collaborative approach, monitoring and directing the care team in their duties. Conduct orientation for new nursing staff and perform regular performance evaluations. Facilitate ongoing education and mentoring for nursing staff, addressing performance issues as necessary.
- Serve as a role model by demonstrating professionalism, empathy, and respect in all interactions, while being mindful of the impact on team dynamics.
- Establish and maintain supportive relationships with Residents, families, and team members through effective communication and interpersonal skills.
- Ensure adequate staffing levels and oversee nursing team collaboration to maintain high-quality, Resident-centered care, addressing any issues that arise.
- Collaborate with leadership to spearhead quality improvement initiatives and performance measurement processes, ensuring compliance with best practices and accreditation standards.
- Focus on quality assurance and safety improvement by actively participating in site committees related to nursing practices and therapeutic protocols.
- Ensure all documentation regarding assessments, observations, and care provided meets professional standards and Park Place policies.
- Lead the nursing team in prioritizing tasks, utilizing resources efficiently, and adapting to changing Resident needs.
- Promote safety within the nursing team by implementing accident prevention strategies and ensuring knowledge of emergency procedures.
- Participate in educational programs to maintain clinical competence and stay updated on best practices.
- Perform additional related duties as assigned.
HEALTH AND SAFETY DUTIES:
- Read, understand, and comply with health and safety policies and practices.
- Educate staff on workplace safety topics, including violence prevention and harassment.
- Demonstrate leadership in promoting safe work procedures and practices.
- Take necessary precautions to ensure the safety of Residents, staff, and the public.
- Report any incidents or unsafe conditions to management promptly.
- Lead by example in maintaining a safe work environment.
QUALIFICATIONS:
Must be a Registered Nurse
SKILLS AND COMPETENCIES:
- Fluent communication in English, both verbal and written.
