About the Role:
We are seeking a skilled Registered Nurse to join our team in a community care setting. As a Registered Nurse, you will have the opportunity to work with patients of all ages, providing scheduled visits to care for patients in various settings.
Key Responsibilities:
- Assess patients' individualized needs using appropriate assessment tools and techniques
- Execute and continuously monitor care plans
- Engage patients using a collaborative approach by building rapport and trust
- Maintain and update professional patient records related to patient encounters and plans

Requirements:
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care is an asset
