About the Role:
VON Canada is seeking a highly skilled and compassionate Registered Nurse to join our team in Ontario. As a Registered Nurse, you will be responsible for providing high-quality, client-centered care to individuals and families in their homes and communities.
Key Responsibilities:
- Manage and coordinate care, evaluating health outcomes and educating clients and families to meet their health goals.
- Provide all levels of nursing care, from stable clients with predictable outcomes to those with acute, complex, and rapidly changing health needs.
- Develop and implement care plans, incorporating discharge planning as needed, and ensuring that services meet client needs.
- Collaborate with clients, families, and caregivers to promote independence and self-care, using adult learning principles and established teaching materials.
- Participate in decision-making regarding client care plans and follow up to promote continuous quality improvement.
- Function in accordance with VON policies, procedures, and strategic direction.
Requirements:
- Licensure as a Registered Nurse in the province of practice.
- Canadian Nurses Association certification in Community Health Nursing is an asset.
- Clinical practice experience and experience in community health are assets.
