As a Mental Health and Addiction Therapist within our Integrated Mood & Anxiety and Addictions Concurrent Program, you will play a key role in a collaborative, multidisciplinary team focused on providing comprehensive services and programs to our clients. Your responsibilities will encompass treatment planning, clinical case coordination, and direct service delivery through both individual and group therapy for individuals facing complex mental health and addiction challenges.

Key Responsibilities:
  • Conduct and coordinate assessments to inform treatment planning.
  • Deliver case management services, including service planning, coordination, navigation, and progress evaluation.
  • Facilitate individual and group psychotherapy utilizing evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, and Cognitive Behavioral Therapy (CBT).
  • Provide support to family members and lead educational group sessions.
  • Collaborate closely with physicians and the clinical team to enhance patient experiences through regular meetings to review progress and adjust treatment plans.
  • Oversee discharge planning and referrals to appropriate community resources.
  • Identify discharge needs based on comprehensive assessments and care plans.
  • Lead the establishment of treatment goals and collaborate with patients and team members to create actionable steps.
  • Monitor and evaluate progress on treatment plans as a team.
  • Act as a communication liaison among patients, the team, and the program, ensuring critical information is shared effectively.
  • Maintain documentation and record-keeping in accordance with organizational and regulatory policies.
Qualifications:
  • Bachelor's or master's degree in a recognized Social Science discipline with specialized education in addiction.
  • Membership in good standing with the relevant regulatory college in Ontario or in pursuit of relevant credentials.
  • Proficiency in delivering CBT, DBT, and other evidence-based approaches for complex mental health and substance use disorders.
  • Demonstrated experience in assessment and treatment using evidence-based methodologies.
  • Exceptional communication and interpersonal skills.
  • Understanding of measurement-based care to guide clinical decision-making.
  • Ability to lead and facilitate an interdisciplinary team effectively.
  • Skills in individual and group counseling, including the capacity to present psycho-education to larger audiences.
  • Commitment to delivering inclusive and culturally responsive care.
  • Knowledge of addiction issues across a continuum of care.
  • Valid Ontario driver's license and access to a vehicle as needed.
  • Familiarity with relevant legislation such as the Mental Health Act and Health Care Consent Act.
