Key Responsibilities
Job Summary
The Home Care LPN is tasked with delivering direct nursing care as directed by the physician to patients who are acutely or chronically ill in their residences. This includes documenting clinical notes and coordinating patient care activities to ensure the highest quality of service in the designated area. The LPN operates under the supervision of a Registered Nurse or designated supervisor, adhering to the State LPN/LVN Practice Act and agency policies.
Specific Duties
- Deliver high-quality clinical services within the defined scope of practice, following the care plan and collaborating with the healthcare team while adhering to infection control protocols.
- Thoroughly document patient visits in compliance with regulatory and payer standards using the agency's electronic medical record system.
- Engage in the agency's care coordination process by promptly reporting patient needs and changes in condition, participating in case conferences, and communicating effectively with patients, families, physicians, and other healthcare professionals.
- Maintain confidentiality of patient health information in accordance with agency policy.
Qualifications
Licensure Requirements
- Must possess a current LPN license in the state of practice.
- Valid driver's license, vehicle insurance, and access to reliable transportation.
- Current CPR certification is mandatory.
Educational Requirements
- Must fulfill educational and experience criteria as outlined in state licensure laws.
Experience Requirements
- Prior experience in home care is preferred.
Skill Requirements
- Ability to connect with and care for acutely and chronically ill patients and their families.
- Strong patient management and documentation skills are essential.
