Long-Term Care Registered Practical Nurse (RPN)

At Southbridge Care Homes, we are committed to delivering exceptional care to our residents in long-term care settings. We are seeking a dedicated RPN who is enthusiastic about enhancing the quality of life for our residents through compassionate and skilled nursing practices.

Primary Responsibilities:

  • Oversee and execute best practices within the Resident Assessment Instrument-Minimum Data Set (RAI-MDS) framework
  • Collaborate with an interdisciplinary team to ensure comprehensive resident care
  • Facilitate training and educational sessions on RAI MDS protocols
  • Maintain high standards of resident care while adhering to established guidelines
  • Monitor, document, and report on vital signs and health changes
  • Coordinate and manage activities related to resident care
  • Support the leadership and management of nursing personnel

Qualifications:

  • 3-5 years of experience in Long-Term Care settings
  • Active Registration with the College of Nurses of Ontario (CNO) in good standing
  • Familiarity with nursing rehabilitation practices and collaboration with physiotherapy
  • Strong analytical and assessment capabilities
  • Proven ability to conduct training and education for adult learners
  • Knowledge of the InterRAI suite of assessment tools and patient classification systems
  • Excellent communication skills
