As a key member of our Mental Health and Addictions Program, you will be responsible for coordinating and developing case management services for our clients. Your expertise will be essential in facilitating screening and assessment, providing case management services, and promoting community linkages.
Key Responsibilities:
- Collaborate with our interdisciplinary healthcare team to develop individualized patient care plans.
- Provide case management services to clients, including crisis intervention and counseling.
- Maintain accurate patient care documentation and statistics.
- Advocate on behalf of patients and families.
- Act as a navigator for regional psychiatry services and direct patients to sources of support.
Qualifications:
- Post-secondary education in Social Service Work, Social Work, or equivalent.
- Registration with Ontario College of Social Workers and Social Service Workers, or eligible to obtain.
- Excellent analytical and problem-solving skills.
- Ability to work in a dynamic and changing environment.
- Strong communication and interpersonal skills.
Salary: Commensurate with qualifications and experience.
