Job Summary

The Personal Support Worker provides personalized care and support to clients in their homes, promoting independence and well-being. This role involves collaborating with healthcare teams to develop and implement care plans, providing assistance with daily living activities, and ensuring client safety and comfort.

Key Responsibilities
  • Conduct home assessments to identify client needs and develop personalized care plans
  • Provide assistance with daily living activities, such as bathing, dressing, and grooming
  • Prepare nutritious meals and snacks, and assist with light housekeeping
  • Monitor and maintain household safety, and report any concerns to supervisors
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery
  • Participate in multidisciplinary teams to develop and implement care plans
  • Document client progress and report any changes or concerns to supervisors
Requirements
  • High school diploma or equivalent
  • Personal Support Worker program or Health Care Aide certification
  • Current Standard First Aid Certificate or CPR level C Certificate
  • Experience working with elderly or ill individuals or children
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
