Job Summary: The Health Care Aide at Phoenix Society operates under the guidance of a Licensed Practical Nurse or designated professional, collaborating with the interdisciplinary team to facilitate client care in alignment with established policies, standards, and procedures.

Work Performed Client Services:
  • Build and sustain therapeutic relationships with patients, clients, and families, offering support in accordance with established care plans.
  • Welcome patients, assist with the organization of personal belongings, and create a safe and comfortable environment while ensuring confidentiality and respect for patient rights.
  • Support nursing care delivery under the supervision of a Licensed Practical Nurse or designate, performing tasks such as monitoring vital signs, administering dressings, and assisting with personal hygiene needs.
  • Observe and document patient/client progress, symptoms, and behavioral changes, reporting significant findings to the Licensed Practical Nurse or designate.
  • Respond to call bells and alarms, alerting the appropriate health care team member and assisting as needed, while managing emergent situations according to unit guidelines.
  • Utilize mechanical aids for client transport and provide assistance with positioning and transferring clients using established safety techniques.
  • Retrieve lab results and patient information from external systems as required.
  • Contribute suggestions for enhancing client care and improving operational efficiency.
Operations:
  • Ensure a clean and safe environment by maintaining equipment, managing hazardous materials, and reporting incidents or safety concerns.
  • Prepare rooms for discharge cleaning and assist in room setup, including laundry management and supply organization.
  • Clean shared equipment and maintain supplies in various areas, ensuring adequate stock levels.
  • Participate in team meetings and training sessions to enhance collaborative practices.
Administration:
  • Perform clerical duties such as answering phones, providing general information, and maintaining patient flow.
Qualifications:
  • Post-secondary diploma in nursing assistance or a related field (2 to 3 years).
  • 1 to 3 years of experience in trauma-informed acute or community health nursing care for patients with diverse needs.
  • Current registration with the BC Care Aide Registry is mandatory.
  • Successful completion of a criminal records check with vulnerable sector screening is required.
  • Completion of Tuberculosis Screening and Employee Immunization Record forms is necessary.
  • First Aid Level 1 and current CPR certification are required.
  • A valid driver's license is essential.

  • Computer literacy for operating patient care information systems and related software.
