Job Summary

We are seeking a highly skilled and compassionate Clinician to join our team at Fraser Health. As a key member of our multidisciplinary team, you will provide assessment, treatment, consultation, and follow-up services to mentally ill elderly clients of special complexity and difficulty, and their caregivers.

Key Responsibilities
  1. Client Assessment and Treatment: Conduct comprehensive psychosocial assessments, provide treatment planning and interventions using a variety of psychosocial treatment modalities, and offer crisis intervention and family counseling through home visits and outreach.
  2. Collaboration and Liaison: Participate as a member of the interdisciplinary team, coordinate work, and cooperate with members of the team, including attendance at team meetings and relevant conferences.
  3. Documentation and Reporting: Maintain clinical and statistical documentation according to established policies and procedures, and prepare written reports as required.
  4. Community Engagement: Facilitate client and caregiver utilization of appropriate community services, provide consultation and support to families and other informal caregivers, and liaise with community agencies to ensure seamless care coordination.
  5. Advocacy and Education: Act as an advocate on behalf of clients and their families, provide consultation and education to referral agencies, care facilities, and caregivers, and represent the team on local committees and advisory boards as required.
Requirements
  • Education and Experience: Master's degree in Social Work, Clinical Counselling, or other relevant health-related discipline, with one year's recent experience working with geriatric psychiatry clients in a recognized psychiatric and/or community mental health setting.
  • Skills and Abilities: Demonstrated clinical skills in working with older adults with age-related mental health issues, ability to communicate effectively with professionals, clients, and caregivers, and ability to work effectively within interdisciplinary teams.
