We are seeking a highly skilled Health Care Social Worker to join our team at the Nova Scotia Health Authority. As a key member of our interdisciplinary team, you will provide comprehensive social work services to patients and their families, promoting optimal health and well-being.
Key Responsibilities- Conduct thorough psychosocial assessments to identify patients' needs and develop individualized care plans.
- Provide counseling and therapy services to patients and their families, addressing issues related to mental health, substance abuse, and other social determinants of health.
- Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination and transitions.
- Develop and implement community-based programs and services to promote health and well-being, particularly for vulnerable populations.
- Provide education and support to patients and their families on health-related topics, including disease management and self-care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in social work.
- Master's degree in Social Work from a recognized university.
- Registration or eligibility for registration with the Nova Scotia College of Social Workers.
- Two years of clinical experience as a social worker, preferably in a healthcare setting.
- Strong counseling and communication skills, with the ability to work effectively with diverse populations.
- Capacity to work autonomously and collaboratively within an interdisciplinary team.
- Knowledge of issues related to family violence, sexual abuse, addictions, grief and loss, end of life, disability, crisis intervention, caregiving, and continuum of care.
- Competencies in other languages an asset, particularly French.
- Long assignment, full-time position, 75 hours bi-weekly.
- Approximately an 18-month assignment.
We offer a competitive salary range of $37.31 - $49.05 hourly, as well as a comprehensive benefits package, including health, dental, travel, long-term disability, and life insurance coverage, and a defined benefit pension plan.
