Key Responsibilities:

- Provide education and support to patients and families to promote self-management and independence.
- Identify and address social determinants of health that impact patient outcomes.
- Develop and maintain relationships with community partners to ensure access to necessary resources.
- Participate in quality improvement initiatives to enhance patient flow and hospital efficiency.
Requirements:
- Masters degree in Social Work or related field.
- Current registration with a regulated college.
- 2-5 years of experience in acute care or discharge planning.
