Social Worker/Discharge Planner

Job Summary:
We are seeking a highly skilled Social Worker/Discharge Planner to join our interdisciplinary team at Trillium Health Partners. As a key member of our Discharge Planning team, you will play a critical role in supporting patients and families to make timely transitions through the hospital system.
Key Responsibilities:
- Employ critical thinking and creative problem-solving to facilitate discharge planning, including complex situations.
- Provide in-depth discharge-focused assessments, collaborating with the team, patient, and family to create a discharge plan and provide support, education, and guidance.
- Facilitate timely transitions to the community, identifying safety gaps and addressing barriers using appropriate resources.
- Assess risk and need for psychosocial intervention, providing psychosocial assessments and referrals as needed.
Requirements:
- Masters degree in Social Work preferred.
- Bachelors degree in a Health Discipline required.
- Current registration and membership with a Regulated College.
- 2-5 years clinical experience in an acute care hospital setting.
- Experience in Discharge Planning is essential.
- Demonstrated ability to conduct psychosocial assessments and counseling, as well as assess risk and need for referral to Social Work.
- Excellent interview skills and ability to work independently and collaboratively with an interdisciplinary team.
