Social Worker

Fraser Health is seeking a skilled Social Worker to join our team at Chenchenstway in Burnaby, BC. As a Social Worker, you will play a vital role in supporting residents and their families to adjust to the stress associated with chronic illness/disability, relocation, and institutional life.
Responsibilities- Conduct psycho-social assessments of residents and clients, and participate in pre-admission assessments as required.
- Provide counseling services to residents/clients, families, staff, and groups, and make referrals to appropriate community agencies as needed.
- Attend interdisciplinary care conferences and consult with others internally and externally regarding resident and client services.
- Assist in the development, implementation, and review of resident and client care plans, and record actions taken on relevant health records.
- Support the needs assessment, development, provision, and evaluation of group work for residents/clients and families.
- Determine residents'/clients' eligibility for financial assistance, advise residents/clients and families, and make referrals as necessary.
- Participate in resident and client discharge planning.
- Attend meetings and participate in hospital and community planning as assigned, and assist with research projects and education programs as needed.
- Develop and provide relevant education programs for residents/clients, families, and staff Social Workers.
Education, Training, and Experience:
Bachelor's Degree in Social Work from an accredited school of Social Work. Two years' recent, related experience working as a Social Worker in a long-term care setting, inclusive of clinical training and experience in individual, marital/family, and group modalities of social work practices.
Current full registration with the BC College of Social Workers.
Skills and Abilities:
- Ability to deal with residents, staff, and the public in a tactful and courteous manner.
- Ability to work in a multi-disciplinary team setting.
- Ability to work with frequent interruptions and to organize and prioritize workload.
- Ability to communicate effectively both verbally and in writing.
- Must possess physical ability necessary to perform the duties of the position.
