Job Title: Personal Support Worker

CarePartners is seeking a skilled Personal Support Worker to join our team in the Lindsay area. As a valued member of our interdisciplinary care team, you will work with patients and their families to provide high-quality patient care and support.

Job Summary:

We are looking for a compassionate and dedicated Personal Support Worker to provide care and support to patients in their homes. The successful candidate will have excellent interpersonal skills, be able to work independently and as part of a team, and have experience working with patients with complex care needs.

Responsibilities:
  • Assist patients with daily living activities, such as personal care, household tasks, and medication assistance
  • Provide cognitive, social, and emotional support to patients
  • Prepare nutritious meals and snacks, using proper food preparation and hygiene practices
  • Provide patient status reports and participate in patient/family care planning meetings
Requirements:
  • Valid driver's license and vehicle for which you are insured
  • Community Support Worker (e.g. Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program) education, with preference given to those with a PSW certificate and/or equivalency
  • Current vulnerable sector check screening, and a clear background check
