Your Opportunity:

This position is integral to the Integrated Home Care Program, reporting directly to the Area/Care Manager. The HCA Supervisor is a vital member of an inter-professional health care team, primarily responsible for overseeing Health Care Aides (HCAs). This role entails accountability for personal practice in accordance with the standards set by the College of Licensed Practical Nurses of Alberta. The HCA Supervisor is tasked with assessing, planning, monitoring, and evaluating the care needs of Home Care clients and their families, in collaboration with the RN Case Manager for both Short Term and Long Term care.

The role may require working across various environments, including residential care, assisted living, home care clinics, and community settings. Key responsibilities include:

  • Supervising HCAs (Health Care Aides).
  • Assessing, planning, organizing, and evaluating client needs and goals for acute or rehabilitative care.
  • Traveling to designated areas as required.

Description:

As a Licensed Practical Nurse (LPN), you will collaborate within an interprofessional health care team, employing nursing processes, critical thinking, and problem-solving skills. Your practice will be guided by your education, experience, and the policies and procedures of Alberta Health Services. You will play a crucial role in delivering safe, quality, patient-centered care, embodying the vision and values of the organization.

Required Qualifications:

To qualify, you must have:

  • Completion of an accredited practical nurse education program.
  • Active or eligible registration with the College of Licensed Practical Nurses of Alberta (CLPNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications:

Further qualifications include:

  • Completion of Continuing Care Case Management.
  • Demonstrated competency in personal and professional accountability, systems thinking, and effective communication.
  • Successful completion of a recognized Practical Nursing educational program.
  • Completion of a Medication Administration Course for LPNs.
  • Recent experience as a Licensed Practical Nurse, including supervisory experience.
  • Experience in community health is a definite asset.
  • A strong interest in wound care and a commitment to ongoing education and training.
  • Excellent interpersonal, communication, and organizational skills.
  • Physical capability to perform job-related tasks.

Preferred Qualifications:

This position provides support across multiple home care sites, necessitating travel to various locations as part of the role.
