Role Overview

As a Mental Health and Addiction Recovery Facilitator, you will play a crucial role in supporting individuals facing mental health and substance use challenges. Your responsibilities will include:

  • Observing and documenting client behaviors and symptoms in alignment with treatment goals, ensuring accurate reporting of any changes in clients' health and well-being.
  • Engaging in the intake, care planning, and discharge processes by conducting interviews with clients and their families to assess needs, gather relevant histories, and contribute to case meetings.
  • Building trusting relationships with clients, providing emotional support, and assisting them in navigating daily living challenges, including personal care and problem-solving.
  • Aiding clients in their daily activities, including time management and social interactions, while serving as a role model for positive behavior.
  • Collaborating with healthcare professionals and community resources to coordinate services and advocate for clients' needs.

Qualifications and Skills

To be successful in this role, you should possess:

  • A diploma in Social Services Support Work or a related field.
  • A minimum of two years of relevant experience or a combination of education and training.
  • A valid driver's license and the ability to travel locally.

Knowledge and Abilities

The ideal candidate will have:

  • An understanding of recovery principles in mental health.
  • Familiarity with community resources and health services.
  • Insight into substance use issues and the challenges faced by individuals with addiction.
  • The ability to foster rapport with clients and recognize their evolving needs.
