Role Overview
As a Mental Health and Addiction Recovery Facilitator, you will play a crucial role in supporting individuals facing mental health and substance use challenges. Your responsibilities will include:
- Observing and documenting client behaviors and symptoms in alignment with treatment goals, ensuring accurate reporting of any changes in clients' health and well-being.
- Engaging in the intake, care planning, and discharge processes by conducting interviews with clients and their families to assess needs, gather relevant histories, and contribute to case meetings.
- Building trusting relationships with clients, providing emotional support, and assisting them in navigating daily living challenges, including personal care and problem-solving.
- Aiding clients in their daily activities, including time management and social interactions, while serving as a role model for positive behavior.
- Collaborating with healthcare professionals and community resources to coordinate services and advocate for clients' needs.
Qualifications and Skills
To be successful in this role, you should possess:
- A diploma in Social Services Support Work or a related field.
- A minimum of two years of relevant experience or a combination of education and training.
- A valid driver's license and the ability to travel locally.
Knowledge and Abilities
The ideal candidate will have:
- An understanding of recovery principles in mental health.
- Familiarity with community resources and health services.
- Insight into substance use issues and the challenges faced by individuals with addiction.
- The ability to foster rapport with clients and recognize their evolving needs.
- A commitment to understanding and addressing Indigenous rights and anti-racism initiatives.
