Job Summary:

As a Case Manager, you will be responsible for providing comprehensive case management services to the Sacred Care homes, working closely with foster parents, youth, and support teams to ensure the well-being and safety of our clients.

Key Responsibilities:

  • Establish and maintain positive relationships with foster parents, youth, and support teams through regular home visits, communication, and collaboration.
  • Conduct monthly case planning meetings with assigned collaterals to ensure effective service delivery and outcomes.
  • Complete required documentation, including Foster Care Licensing and service records, in a timely and accurate manner.
  • Maintain a thorough understanding of services provided in each home and communicate effectively with the Program Manager and SECFS.
  • Collect, review, and distribute monthly reports and incident reports, assessing concerns, strategizing solutions, and supporting homes in responding appropriately.
  • Identify training needs for House Parents, Support, and Respite Workers and facilitate access to relevant training opportunities.
  • Ensure outcomes, assessments, and service plans are current and up-to-date.
  • Perform other duties as assigned, including regular travel to Winnipeg and surrounding areas.

Qualifications:

  • Bachelor of Social Work (BSW) degree or equivalent experience working in the field.
  • Knowledge of colonization and intergenerational trauma, with a strong understanding of First Nations perspectives, culture, and philosophies.
  • Excellent conflict resolution skills and ability to navigate complex situations.
  • Knowledge of the Child and Family Services (CFS) sector and legislation, as well as foster care standards and related licensing paperwork.
  • Strong proficiency in Microsoft Office, including word processing, spreadsheets, and email.
  • Excellent communication, organization, problem-solving, and team skills.
  • Ability to speak Anishinaabe is an asset.
  • Valid Manitoba driver's license and reliable vehicle with a clear driver's abstract.
