Responsibilities of the Licensed Practical Nurse (LPN):
- Contributes to the establishment and maintenance of nursing practice standards.
- Executes essential nursing skills (evaluation, intervention) in accordance with established guidelines and protocols.
- Engages in interdisciplinary committees that shape policies impacting nursing practices.
- Adheres to facility protocols for admitting residents and managing transfers to healthcare facilities, ensuring proper documentation is completed.
- Conducts rounds in resident rooms, ensuring that L.P.N.'s, C.N.A.'s, and other staff receive necessary reports and have their assignments organized.
- Assists L.P.N. in creating routines that promote high-quality nursing care.
- Facilitates communication of reports between outgoing and incoming shifts, ensuring accurate counting of controlled substances.
- Coordinates and prepares MDS schedules for assessments on a quarterly, annual, and significant change basis.
- Completes MDS Booklet according to the established schedule (Nursing Sections).
- Reviews and approves all finalized MDS and RAP Summary documents.
- Develops individualized care plans that reflect an understanding of resident-specific needs based on health conditions.
- Modifies and updates care plans as necessary.
- Ensures the implementation of care plans, demonstrating knowledge of prioritization based on expected outcomes.
- Oversees the delivery of care for residents within the assigned unit.
- Communicates the philosophy and objectives of nursing services to residents, families, and other stakeholders.
- Identifies educational needs of residents and families based on discharge plans and provides necessary instruction.
- Exhibits the ability to collaborate effectively with residents, families, and other members of the healthcare team.
- Monitors the administration and accurate documentation of medications.
- Investigates and addresses medication discrepancies.
- Informs physicians of relevant observations, incidents, and abnormal lab and imaging results promptly.
- Transcribes physician orders accurately and ensures timely implementation.
- Reports all incidents involving residents, visitors, and staff, ensuring thorough investigation and documentation.
- Welcomes guidance and constructive feedback as necessary.
- Practices in accordance with the Nurse Practice Act and relevant regulatory standards.
- Maintains adequate stock levels of medications, medical supplies, and equipment at all times.
- Assumes responsibility and accountability for directing nursing shifts.
- Performs additional duties as assigned by the Director of Nursing Services or Administrator.
Qualifications/Requirements for the Licensed Practical Nurse (LPN):
- Graduate of an accredited Nursing School.
- M.S./B.S. or equivalent experience preferred.
- Demonstrated nursing experience with clinical competence and strong leadership abilities.
- Understanding and appreciation of the aging process.
- Ability to work independently in designated areas of responsibility.
Must be a Canadian citizen
Salary: Up to $40 per hour
