Job Summary:
The Sioux Lookout Meno Ya WIn Health Centre seeks a skilled Nurse Practitioner to join our interdisciplinary team. As a key member of our healthcare team, the successful candidate will be responsible for delivering high-quality diabetes education and management services to clients in outpatient and community settings.
Key Responsibilities:
- Develop and implement diabetes education programs in accordance with Diabetes Canada best practices and patient-centered care principles.
- Collaborate with healthcare professionals to provide comprehensive care to clients.
- Conduct individual and group sessions to educate clients on diabetes management and prevention.
- Provide foot care services and conduct regular assessments to prevent complications.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care.
Requirements:
- Current registration with the College of Nurses of Ontario.
- Current Basic CPR certificate (BCLS) required.
- Advanced foot care certification preferred.
- Minimum one year experience in a healthcare setting.
- Certified Diabetes Educator (CDE) designation preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and crisis intervention skills.
Working Conditions:
- Must be physically capable of performing duties of position.
- Occasional evening or weekend work required.
- Must travel north a minimum of 1 week per month.
Salary: $39.07-56.00 in accordance with ONA Collective Agreement
