Nurse Practitioner

ParaMed Home Health Care is seeking a skilled Registered Nurse to provide high-quality community nursing care to patients in various settings.
As a Registered Nurse with ParaMed Home Health Care, you will have scheduled visits to care for patients of all ages with differing complexity of care needs.
Key Responsibilities:- Conduct thorough assessments of patients' individualized needs using appropriate assessment tools and techniques
- Develop and continuously monitor care plans to ensure optimal patient outcomes
- Build strong relationships with patients and their families through a collaborative approach
- Maintain accurate and up-to-date professional patient records related to patient encounters and plans
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a positive impact in your community
- Willingness to share in on-call coverage to ensure patients receive timely care
