About the Role

We are seeking a skilled Life Skills Outreach Worker to join our team at the Interior Health Authority. As a member of our Mental Health and Addictions Team, you will provide comprehensive services to adults with serious and persistent mental illness and/or substance use problems.

Key Responsibilities
  • Provide outreach, community, and facility care services to seniors with serious and persistent mental health issues and/or substance use problems.
  • Work collaboratively with a multi-disciplinary team to provide a comprehensive array of services within a psychosocial rehabilitation framework.
  • Develop and implement individualized treatment plans to support clients in achieving their goals and improving their quality of life.
  • Provide education and support to clients and their families on mental health and addiction issues.
  • Participate in regular team meetings and contribute to the development of team goals and objectives.
Requirements
  • Post-secondary education in a related field, such as social work, psychology, or nursing.
  • Experience working with adults with serious and persistent mental illness and/or substance use problems.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
