Interior Health Authority is seeking a skilled Life Skills Outreach Worker to join our team at the Lansdowne Centre in Kamloops.

About the Role:

This permanent full-time position will provide outreach services to seniors with serious and persistent mental health issues and/or substance use problems/disorders. The successful candidate will work as part of a multi-disciplinary Mental Health and Addictions Team, providing a comprehensive array of services within a psychosocial rehabilitation framework.

Key Responsibilities:
  • Provide outreach services to seniors in the community, including assessment, planning, and implementation of individualized support plans.
  • Work collaboratively with other team members to provide a comprehensive array of services, including outreach, community, and facility care.
  • Develop and maintain relationships with community partners and stakeholders to ensure effective service delivery.
  • Participate in ongoing professional development and training to enhance skills and knowledge in mental health and addictions support.
Requirements:
  • Diploma or degree in a related field, such as social work, psychology, or a related field.
  • Experience working with seniors with mental health and/or substance use issues.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on providing high-quality services to clients.
