Wellness Coordinator

We are seeking a highly skilled and compassionate Wellness Navigator to join our Community Health Team. As a key member of our team, you will play a vital role in supporting citizens of our communities in accessing health-enhancing services, programs, and resources.
The successful candidate will demonstrate flexibility and innovation in providing guidance and support to clients, including information about appropriate supports and programs that meet their health and wellness needs. You will work collaboratively with our team, family physicians, other healthcare providers, and community groups to maintain awareness of existing supports within the community and surrounding areas.
We are looking for a candidate with a strong background in health care, excellent interpersonal and written communication skills, and the ability to work effectively in a team environment. If you have a passion for health promotion and community development, we encourage you to apply.
About You- Bachelor's degree required from an accredited institution in a health care discipline such as Occupational Therapy, Social Work, Registered Nurse, or Recreation Therapy.
- Registration with the respective professional regulatory association required.
- Minimum of 3 years' experience in clinical practice, working in a Primary Health Care setting, community-based team, or community setting, with a minimum of one year recent mental health experience.
- Facilitation/education skills utilizing adult education, self-management supports, and motivational enhancement principles.
- Experience working with people living with chronic conditions.
- Experience in partnership development, stakeholder collaboration, and community development.
- Understanding of population health, health promotion strategies, and behaviour change principles.
- In-depth knowledge of local health authority and community-based services.
- Demonstrates person-centered/family-focused, diversity relevant approaches to care.
- Well-developed interpersonal, teamwork, oral and written communication skills.
- Demonstrates flexibility, adaptability, and willingness to change.
- Demonstrates innovation in program planning, implementation, and evaluation, and quality improvement.
- Computer skills (MS Office, email, and internet).
- Annual CPR-AED certification and annual WHMIS training and certification.
- Valid driver's license and daily access to a reliable car for travel for work.
- Competencies in other languages preferred, French or Arabic is an asset.
- Permanent part-time position (0.60); 45 hours bi-weekly.
- Shifts may include some evenings, weekends, and holidays.
$42.89 Hourly.
