ResponsibilitiesAssessment, Diagnosis, and Treatment- Conduct comprehensive health assessments to identify health concerns and risks.
- Develop and implement treatment plans based on sound clinical reasoning and evidence-based practice.
- Order and interpret diagnostic tests, prescribe medications, and perform minor invasive procedures as needed.
- Collaborate with patients, families, and other healthcare professionals to share decision-making and set priorities for care.
- Communicate effectively with the healthcare team to ensure coordinated and comprehensive care.
- Act as a clinical expert and resource to empower client care delivery.
- Apply principles of pharmacology to select and prescribe medications as part of the treatment plan.
- Monitor and discuss patient response to medication therapy and make adjustments as necessary.
- Utilize a population health approach to plan, develop, and implement health promotion, illness prevention, and health management strategies.
- Collaborate with clients, interdisciplinary teams, and community members to assess health promotion and illness prevention needs.
- Document clinical care in a timely, accurate, and relevant manner.
- Complete statistical data and reports as required.
- Participate in research relevant to practice and utilize research findings in clinical practice.
