Primary Care Nurse Practitioner Opportunity

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at the Flowerstone Family Health Clinic in Qualicum Beach, British Columbia. As a key member of our inter-professional team, you will play a vital role in delivering high-quality, patient-centered primary care to the Oceanside community.

About the Role

The successful candidate will be responsible for providing comprehensive primary care services to patients in the Oceanside area, including routine check-ups, health assessments, and management of chronic conditions. You will work closely with our team of medical office assistants, registered nurses, and clinical pharmacists to ensure seamless care coordination and patient outcomes.

Key Responsibilities
  • Implement processes to ensure all people in the community have access to quality primary care and are attached to a primary care provider within their community.
  • Deliver extended hours of care, including early mornings, evenings, and weekends.
  • Provide same-day access for urgently needed primary care through the NP-PCC.
  • Collaborate and coordinate care with diagnostic services, hospital care, specialty care, and community services for all patients.
