Job Description

Care and Support for Our Community

We are seeking a skilled Speech-Language Pathologist to join our team at Home and Community Care Support Services. As a valued member of our team, you will play a crucial role in providing high-quality care and support to our clients in their homes and communities.

Key Responsibilities:
  • Assessment and Treatment Planning: Conduct comprehensive assessments of clients' speech and language needs, and develop personalized treatment plans to address their goals and objectives.
  • Therapy Services: Provide one-on-one therapy sessions to clients in their homes, schools, and nursing homes, using evidence-based practices and techniques.
  • Collaboration and Communication: Work closely with clients, caregivers, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Documentation and Reporting: Maintain accurate and detailed records of client progress, and provide regular reports to stakeholders as required.
  • Professional Development: Stay up-to-date with best practices and industry developments through ongoing education and training.
Requirements:
  • Education and Certification: Master's degree in Speech-Language Pathology and current certification with the College of Audiologists and Speech-Language Pathologists of Ontario.
  • Experience: At least two years of experience in a similar community-based setting, with a strong understanding of healthcare systems and policies.
  • Skills and Abilities: Excellent communication and interpersonal skills, with the ability to work effectively in a team environment. Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple clients.
