Job Summary:

The Darling Home for Kids is seeking an experienced Social Worker to provide direct social work services to families of children with medical complexities. This role involves providing psycho-educational, supportive, and solution-focused approaches in individual, family, and group counseling to address challenges consistent with respite, residential, and hospice palliative care.

Key Responsibilities:

  • Provide direct social work services to children, adolescents, and families of children who are medically fragile and technology-dependent.
  • Advocate, educate, and provide consultation to meet the complex needs of clients, families, and members of the healthcare team.
  • Develop, implement, and oversee all aspects of a comprehensive and formal grief and bereavement program designed to meet the needs of clients, parents, siblings, extended family, staff, and volunteers.
  • Assess psychosocial needs, develop a plan of care, formulate goal-oriented social work treatment plans and interventions, provide client/family education and support groups pertaining to psychosocial, emotional, and financial needs of clients and families, and document assessments, plans, interventions, and outcomes.
  • Provide service navigation support, maintain knowledge of available resources in the community, and disseminate information to families.
  • Oversee projects relating to the support and education of clients and families, such as hosting education sessions, developing resources for families, and creating informal and formal support networks.
  • Plan and finalize projects in accordance with approved deadlines and budgets by utilizing best practices, analyzing user requirements, and required resources.
  • Support a multi-disciplinary project team through all phases of each project life cycle, providing coaching, motivation, and guidance throughout.
  • Monitor and report on progress, problems, and solutions of projects to all stakeholders, and escalate issues as appropriate.
  • Participate in the creation and delivery of innovative presentations and proposals relating to social supports for families and available resources.
  • Liaise with internal and external stakeholders, including community, regional, provincial, and/or private sector partners.
  • Maintain responsibility and accountability for decisions and actions in the provision and management of support programs within the scope of practice of Social Work as defined by OCSWSSW and as a collaborative member of the interdisciplinary team.

Qualifications:

  • Membership in OASW and registration with OCSWSSW required.
  • M.S.W. experienced in working with children, adolescents, and their families, as well as individual, couple, and family counseling strongly preferred.
  • Ideal candidate must have an undergraduate degree in business, life sciences, health informatics, or related discipline.
  • A Project Management Professional (PMP) designation would be considered an asset.
  • Experience in project management in healthcare environments such as hospitals, government, or private sector is considered an asset.
  • Clinical pediatric experience in either inpatient or outpatient settings preferred.
  • Knowledge of all appropriate legislation is essential.
  • Superior analytical, critical thinking, and problem-solving abilities.
  • Demonstrated ability to anticipate needs and be proactive.
  • Demonstrated effectiveness at leading and working with teams.
  • Experience organizing and facilitating meetings with multiple stakeholders.
  • Effective interpersonal communications skills and ability to establish strong rapport.
  • Demonstrated ability to develop effective and comprehensive reports and presentations for various audiences.
  • Superior writing skills.
  • Excellent organizational, time management, and project management skills.
  • Demonstrated ability to multi-task and prioritize tasks effectively.
  • Proficiency in Excel, PowerPoint, and Microsoft suite skills.
  • Flexible schedule is required to accommodate weekend and evening meetings and sessions.
  • Unlimited access to a reliable vehicle and a valid Driver's License.
