We are seeking an experienced Support Professional to provide individualized support to a woman living with a disability. The successful candidate will work closely with our team to develop and implement personalized support plans, promoting the individual's independence, dignity, and community participation.

**Key Responsibilities:**

* Provide support and assistance to the individual in their daily life, including personal care, household tasks, and community engagement
* Develop and maintain positive relationships with the individual, their family, and other stakeholders
* Collaborate with our team to identify and address the individual's needs and goals
* Participate in training and professional development opportunities to enhance skills and knowledge
* Maintain accurate records and reports as required

**Requirements:**

* Experience working with adults with intellectual disabilities
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Reliable transportation and a safe driving history
* Availability to work 20 hours per week, including Mondays and Tuesdays to Fridays
