The compensation range for this role is CAD $41 to CAD $55. We are seeking a dedicated Primary Care Nurse to become an integral part of our healthcare team.
Role Responsibilities:As a Primary Care Nurse, your responsibilities will include:
- Conducting triage, assessments, planning, implementation, and evaluation of nursing care for clients and families requiring urgent attention.
- Establishing and nurturing collaborations with primary care providers in alignment with the local Community of Care service delivery model.
- Utilizing approved triage standards to apply critical thinking skills within the safe parameters of patient care.
- Implementing chronic disease management, palliative care, health management, and self-care management principles to perform assessments, identify issues, provide direct care, and address any deviations from the client's care plan.
- Serving as a resource for the interdisciplinary team by offering insights based on nursing theory and practice related to patients with primary care needs.
Urgent and Primary Care Centres (UPCC) are designed to cater to individuals with unexpected, non-life-threatening conditions requiring medical attention within a short timeframe. This includes conditions such as sprains, minor cuts, infections, and mild asthma attacks.
UPCCs are intended to complement existing community services, ensuring that patients receive appropriate urgent care when necessary.
Qualifications:Candidates should possess:
- A minimum of two years of recent clinical experience relevant to the designated clinical area, or an equivalent combination of education, training, and experience.
- Proven ability to apply critical thinking skills within safe limits of patient care.
- Knowledge and skills in primary care, along with a patient and family-centered approach.
- Competencies in accordance with the Community Health Nurse of Canada standards.
- Understanding of provincial regulations and guidelines related to home, community, and primary care.
- Expertise in managing acute and chronic diseases, mental health, substance use, and palliative care.
- Ability to provide culturally safe care to diverse populations.
- Experience in Trauma Informed Practice, Harm Reduction, and Recovery Oriented Care.
- Capacity to work independently and prioritize care effectively.
- Knowledge of adult education principles and the ability to educate and coach patients and families.
- Certification in CPR techniques.
