Geriatric Primary Care Specialist

Job Summary:
The Geriatric Primary Care Specialist will provide comprehensive geriatric assessments and supportive care to clients and families in the community environment. This role is part of the multidisciplinary team for the Home Dementia Care Program, a pilot project designed to provide enhanced support to individuals living in the community with moderate to late stage dementia.
Key Responsibilities:
- Complete community-based comprehensive geriatric assessments and develop strategies to reduce falls, prevent delirium, and maintain optimal chronic disease management in the home.
- Function as a practitioner, consultant, educator, leader, and researcher in close affiliation with the local geriatric team.
- Integrate advanced knowledge and experience in delivering safe, effective, and quality care to geriatric clients with dementia in a community, home-based setting.
- Advocate within the healthcare delivery system on behalf of clients with dementia in primary care.
- Liaise with the healthcare delivery system and community partners to optimize geriatric dementia care.
- Assist in the development, promotion, and implementation of best practice in geriatric primary care for dementia.
- Interpret and communicate results of investigations to clients, families, and primary physicians when appropriate.
- Practice prescriptive authority within the scope of practice for the Geriatric Primary Care Specialist.
- Practice with autonomy and collaboration to perform other related duties as required.
Requirements:
- Graduation from a nurse practitioner program approved by Central Health.
- Current registration as a nurse practitioner with the Association of Registered Nurses of Newfoundland and Labrador.
- Current BLS for healthcare providers from the Heart and Stroke Foundation.
- Gentle Persuasive Approach or willingness to complete the next available course.
- CNA certification in Gerontology or willingness to complete the next available course.
- Advanced and current knowledge in comprehensive geriatric assessments, dementia assessments, and proficient knowledge in differential diagnosis of dementia, delirium, and depression.
- Possess strong abilities in the areas of collaboration, clinical reasoning, professional development, interpersonal effectiveness, conflict management, and teamwork.
- Possess strong communication, problem-solving, decision-making, and teaching skills.
- Demonstrated progressive clinical leadership skills.
- Demonstrated organizational and time management skills.
- Demonstrated ability to function autonomously in an advanced practice role.
- Demonstrated ability to support and participate in research to improve practice and dementia care outcomes.
- Demonstrated skills in computer, electronic charting, and database management.
- Consistent demonstration of the core values of NL Health Services: Innovation, Compassion, Accountability, Respect, and Excellence (I CARE).
- Use of a private vehicle is required to visit clients in their home setting.
- A satisfactory record of work performance and attendance is required for this position.
- A satisfactory Certificate of Conduct from the RCMP or local police authority.
Preferred Qualifications:
- Demonstrated skill in using healthcare-based computer systems, CRMS, and Meditech.
Working Conditions:
- 75 hours bi-weekly (Shift work may be required)
- NS-35 ($62.40)
