Job Summary:

The successful candidate will provide high-quality patient care and support to our clients in their homes, ensuring their physical and emotional needs are met.

Responsibilities:
  • Assist patients with daily living activities, such as bathing, dressing, and grooming
  • Provide personal care, including toileting, feeding, and transferring patients
  • Monitor and report any changes in patients' condition to the healthcare team
  • Assist with medication administration and maintain accurate records
  • Prepare and serve meals and snacks, and maintain a clean and safe environment
  • Transport patients to appointments and engage in activities to promote their physical and mental well-being
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Participate in ongoing education and training to enhance skills and knowledge
Requirements:
  • College diploma or certificate in a related field (e.g., nursing, healthcare, social work)
  • Minimum 2 years of experience in a healthcare setting
  • Current First Aid and CPR certification
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to lift, bend, and move patients as required
