Fraser Health is seeking a skilled Psychiatric Liaison Nurse to join our team. As a key member of our interprofessional care team, you will play a vital role in providing person-centered care to patients with psychiatric and concurrent disorders.
Key Responsibilities- Conduct comprehensive psychosocial assessments and develop individualized care plans to address the unique needs of each patient.
- Collaborate with emergency department staff, mental health professionals, and community service providers to ensure seamless transitions of care.
- Provide crisis intervention, counseling, and education to patients and their families to promote recovery and well-being.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Graduation from an approved school of nursing or psychiatric nursing.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- One year of recent experience in acute or community mental health and substance use settings.
