Key Responsibilities:
  • Engage and build rapport with our target population, providing a safe and supportive environment for them to thrive.
  • Perform all aspects of the case management process, utilizing the Pathway of Hope approach to identify and address root causes of poverty.
  • Develop and implement personalized goal plans with clients, ensuring regular progress reviews and adjustments as needed.
  • Maintain accurate and detailed case files, adhering to confidentiality standards and best practices.
  • Conduct home visits, community outreach, and provide emotional support and advocacy as required.
  • Collaborate with other local ministry unit personnel to form an interdisciplinary team, working holistically with families to address their unique needs.
  • Effectively utilize computer management systems to record participant documentation, maintain confidentiality, and provide statistical reports.
  • Demonstrate clinical and counseling skills to provide assessment, engagement, and support services to children, youth, and families.
Requirements:
  • Bachelor's degree in a human service field.
  • Minimum 2 years of related experience, including casework services, case file management, and community outreach.
  • Valid Ontario Class "G" Driver's License and a current copy of a Driver's abstract.
  • Excellent oral, written, and electronic communication skills.
  • Good interpersonal skills, self-motivation, and discipline.
