Nurse Practitioner

We are seeking a highly skilled Nurse Practitioner to join our team at Woolwich Community Health Centre. As a Primary Care Specialist, you will play a vital role in providing comprehensive primary health care to our registered client case load.
Key Responsibilities- Work within an inter-disciplinary team to provide wellness care, assessment, diagnosis, and management of episodic illness, and managing chronic disease.
- Develop and implement individualized care plans to meet the unique needs of our clients.
- Collaborate with registered dietitians, chiropodists, nurses, counselors, physiotherapists, and community health team staff to provide comprehensive care.
- Provide group education and individual counseling to clients with chronic diseases, such as diabetes.
- Conduct home visits as required.
- BScN with Primary Health Care Nurse Practitioner certificate and registered with The College of Nurses of Ontario in Extended Class.
- Certified Diabetes Educator certificate, or experience and willingness to complete CDE credential.
- Broad-based nursing experience with 2-5 years in primary health care setting an asset.
- Strong communication, organizational, and issue management skills.
- Excellent interpersonal and problem-solving skills, within the inter-disciplinary team setting and with clients and community members.
- Flexibility to work one evening per month.
- Knowledge of/experience with health issues across the lifecycle.
- Strong facility with computer; previous experience with electronic medical records would be an asset.
- Salary of $101,905 to $122,178 per year (pro-rated to working status) ($52 to $63 per hour)
- 4 weeks paid vacation (pro-rated to working status)
- 18 Sick Days per year (pro-rated to working status)
- 10 Professional Development days per year (pro-rated to working status)
- Enrolment in Healthcare of Ontario Pension Plan (HOOPP)
