This pivotal role involves collaborating with a multidisciplinary team, patients, and their families to ensure seamless and effective discharges from the Acute Rehabilitation Unit (ARU). The coordinator is essential in managing the intricate discharge process and plays a key role in connecting with community resources and programs.
Key Responsibilities
- Provides guidance and support to bedside nurses, healthcare team members, and families in assessing, planning, facilitating, and evaluating discharges for pediatric patients. This includes coordinating discharge preparation activities such as securing necessary equipment and nursing support, making referrals to suitable outpatient, community, and government services, and ensuring a smooth transition from hospital to community care.
- Collaborates with the interdisciplinary team to develop tailored discharge plans for individual patients, orders home supplies as needed, and identifies potential discharge barriers for patients and their caregivers, aiming to resolve any issues related to patient care during the transition.
- Coordinates educational initiatives for patients and caregivers, which involves creating teaching resources, identifying learning needs, developing educational materials, and conducting instructional sessions.
- Supports a family-centered care model by providing education and guidance from admission through discharge, including the development of materials and mentoring bedside nurses.
- Actively participates in interdisciplinary team meetings, including family meetings with community care providers when appropriate, utilizing phone conferences or TeleHealth as necessary.
Qualifications
- Graduation from an accredited School of Nursing with current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- A minimum of three years of recent clinical experience in a tertiary pediatric hospital or an equivalent combination of education, training, and experience.
Skills & Knowledge
- Proven ability in developing nursing care plans and educational strategies.
- Comprehensive understanding of evidence-based nursing practices related to patient acuity and bed utilization.
- Broad knowledge of various healthcare disciplines and their roles in patient care and discharge planning.
- Awareness of critical perspectives and social justice frameworks, including Trauma-Informed Care.
- Commitment to understanding legislative obligations and provincial commitments as outlined in foundational documents.
- Strong asset: Knowledge of the social, economic, political, and historical contexts affecting Indigenous Peoples and familiarity with addressing Indigenous-specific anti-racism and cultural safety initiatives.
