About the Role

We are seeking a highly skilled and experienced Registered Nurse to fill the position of Patient Care Coordinator at Burnaby Hospital. As a key member of our clinical team, you will be responsible for providing clinical supervision, guidance, and support to nursing staff, other clinical team members, and support staff within the assigned unit(s).

Key Responsibilities
  • Apply established criteria and prioritize admissions/intake, leading and facilitating actions related to daily patient flow, plans for patient discharge, and appropriate patient placement/referral.
  • Supervise nursing and nursing support staff, coordinate the work of the clinical team, and monitor the quality of care and the safety of patients.
  • Support quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out patient/family education, and acting as a mentor and resource to staff.
  • Identify learning requirements of staff, provide input into the development of learning objectives, and evaluate the effectiveness of learning opportunities.
  • Act as a point person for patients and families who have concerns or questions regarding their care, follow up as required, and participate in or lead the investigation of complaints.
  • Collaborate with Quality Improvement according to established protocols and participate in quality improvement and risk management activities.
Requirements
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training, and experience.
  • Completion of postgraduate coursework and/or certification applicable to the designated clinical area.
  • Clinical proficiency/expertise in the designated clinical area, ability to supervise and provide leadership and work direction, and ability to coordinate patient assignments and monitor staff workload and patient factors.
