Job Title: Clinical Coordinator, Partnerships

Job Summary: We are seeking a highly skilled Clinical Coordinator to join our Partnerships Team. As a key leader, you will be responsible for overseeing clinical quality at partner sites, collaborating with internal and external teams, and ensuring we exceed customer needs.

Key Responsibilities:

  • Work closely with the program manager to ensure effective and efficient program operations
  • Track and monitor key performance indicators and ensure the program adheres to them
  • Review reports and provide coaching and feedback to the interprofessional team
  • Act as a lead contact and liaison with internal and external parties
  • Summarize complex assessment findings, facilitate case conferences, and ensure consensus amongst the interprofessional team
  • Be a resource for the team on clinical matters and support ongoing education and knowledge translation
  • Perform clinical assessment work within professional scope and provide treatment services as required (rare)

Requirements:

  • Regulated Health Professional registered with relevant regulatory body (College of Occupational Therapist of Ontario, College of Physiotherapists of Ontario)
  • Membership with relevant Association (preferred)
  • Minimum 3 years relevant experience in assessing and treating patients in third-party work with a strong quality improvement focus to third-party payers including WSIB
  • Excellent written and verbal communication skills
  • Demonstrate exceptional relationship management skills, effectively building and nurturing partnerships with internal and external parties to foster collaboration, trust, and mutual success
  • Evidence of continuing education, professional and personal development
  • Accountable, flexible, and self-directed professional with strong time management and organizational skills
  • Exercises initiative and good judgment with ability to multi-task
  • Coordination of WSIB services with appropriate health care and administrative professionals
  • Reviews and interprets reports and facilitates case conferences with clinical teams and WSIB
  • Participation in the collection and analysis of business benchmarks and clinical outcome measures
  • Works as part of an interprofessional team model, providing evidence-based, outcome-oriented care and recommendations
  • Contributes to the overall vision/mandate of the Program
  • Supervision of students as required
