About the Role

VON Canada is seeking a skilled Licensed Practical Nurse to join our team in providing high-quality home care services to our clients. As a LPN, you will be responsible for delivering patient-centered care in a home setting, working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our clients.

Key Responsibilities
  • Client Care
    • Provide nursing care to clients in their homes, including administering medications, wound care, and vital sign monitoring.
    • Assess client needs and develop individualized care plans to meet their unique requirements.
  • Communication and Collaboration
    • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
    • Collaborate with the interdisciplinary team to share knowledge, expertise, and best practices.
  • Documentation and Reporting
    • Accurately document client information, including medical history, medications, and treatment plans.
    • Prepare and submit reports to the healthcare team, as required.
  • Professional Development
    • Participate in ongoing education and training to stay current with best practices and industry developments.
    • Contribute to quality improvement initiatives and share knowledge with colleagues.
Requirements
  • Education and Experience
    • Diploma in Practical Nursing or equivalent.
    • Current licensure as a Licensed Practical Nurse in the province of practice.
  • Skills and Abilities
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Strong problem-solving and critical thinking skills.
  • Work Environment
    • Work in a fast-paced, dynamic environment with a focus on client-centered care.
    • Travel to clients' homes, with occasional use of a personal vehicle.
