Job Summary

We are seeking a skilled Case Manager to join our Home Health Case Management team at the West Vancouver Community Centre. As a Case Manager, you will be responsible for managing a select population of eligible Home Health clients, determining client goals and needs, and assessing client situations to provide effective care and support.

Key Responsibilities

  • Manage a caseload of Home Health clients, assessing their needs and developing individualized care plans.
  • Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to provide comprehensive care and support.
  • Conduct home visits to assess client needs and provide education and support to clients and their families.
  • Develop and implement care plans that address the physical, emotional, and social needs of clients.
  • Monitor client progress and adjust care plans as needed to ensure optimal outcomes.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.

Qualifications

To be successful in this role, you will require:

  • A current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training, and experience.
  • Local area travel requires the use of a personal vehicle. A valid BC Driver's License is required.
