About the Role

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at RhynoCare. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to our patients in a variety of long-term and acute care facilities in the Azilda area.

Key Responsibilities
  • Provide Nursing Care
    • Deliver nursing care in accordance with established policies, physician's orders, and resident plans of care.
    • Utilize sound nursing practice principles to ensure excellent nursing and personal care in assigned home areas.
  • Ensure Resident Safety and Well-being
    • Ensure timely and accessible admissions for residents, including comprehensive orientation to the home and services offered.
    • Ensure resident transfers and discharges are timely and safe.
  • Manage Medications and Supplies
    • Ensure the supply, control, and safekeeping of drugs and narcotics.
    • Ensure protocols are followed on all assigned shifts.
  • Assess, Plan, Implement, and Evaluate Nursing Care
    • Assess the physical, psychological, emotional, spiritual, cultural, and social needs of residents.
    • Plan, implement, and evaluate nursing care to meet resident needs.
Requirements
  • Education and Certification
    • Active registration with the College of Nurses of Ontario.
    • BLS Certificate.
    • Valid First Aid & CPR.
  • Skills and Abilities
    • Able to work with persons with infectious diseases.
    • Well-developed interpersonal and communication skills.
    • Ability to work independently or as part of a team.
  • Other Requirements
    • Proof of current immunizations for Measles, Mumps, Rubella, and Varicella (MMRV).
    • Proof of negative TB skin test within one year.
    • Proof of Hepatitis B.
    • Current Vulnerable Sector Police Check.
    • Ability to work flexible shift times, including eight or twelve hours in length.
    • Ability to 'float' from unit to unit within a hospital, depending on facility need.
    • Reliability to be placed at an agreed-upon facility/unit.
