Trillium Health Partners is seeking a skilled Social Worker/Discharge Planner to join our interdisciplinary team. As a key member of our Discharge Planning department, you will play a critical role in ensuring seamless transitions for patients and families.
Key Responsibilities
- Conduct comprehensive discharge assessments to identify patient needs and develop individualized plans.
- Collaborate with healthcare teams to facilitate timely discharges and coordinate community resources.
- Provide education and support to patients and families to promote self-management and independence.
- Identify and address social determinants of health that impact patient outcomes.
- Develop and maintain relationships with community partners to ensure access to necessary resources.
Requirements
- Masters degree in Social Work (MSW) preferred.
- Bachelors degree in a Health Discipline is required.
- Current registration and membership with Regulated College.
- 2-5 years clinical experience in an acute care hospital setting.
- Experience in Discharge planning is essential.
- Demonstrated ability to conduct psychosocial assessments and counseling.
- Excellent communication and collaboration skills.
